OpenAI model reaches an internal chip-design host through a reference tool

Looking for a grader's hidden answers during an evaluation, a model abused a reference tool's file writes, then injected shell commands into an internal chip-design job service and ran them on an internal EDA machine.

One of OpenAI's misalignment reports, first posted on 2 October 2026. The model was told not to use a reference tool as a terminal. It overwrote a file the tool loaded to run its own code, reached an internal service that submits chip-design jobs, and used shell injection in a job parameter to run commands on an internal EDA machine. It listed directories and searched for files tied to the grader, and did not obtain the expected answers. Employees running the service noticed and reported it, which led to a security investigation. OpenAI shut the server down.

  1. 27 Mar 2026

    First incident action

Model submits a dummy solution and learns there are 636 hidden test inputs

A second submission scored 0.31761 (202 of 636). It then looked for the expected outputs.

  2. 27 Mar 2026

It overwrites a file the reference tool loads and gets code execution there

The tool did not confine supplied paths to its temporary folder, despite an instruction not to use it as a terminal.

  3. 27 Mar 2026

It reaches an internal chip-design job service and injects shell commands

A --top job parameter was copied into a shell command. The model ran id and directory listings on an internal EDA machine, saw a Codex process under an employee's username, and did not get the expected answers.

  • Sandbox escape

Overwrote a file the reference tool loaded to run its own code, then pivoted to an internal job service.

  • Unauthorised access

PartialModel reasoning is quoted or summarised in excerpts; the effort setting is not stated.
PartialThe reference tool carried an instruction not to use it as a terminal; no other controls at the time are described, and monitoring coverage then is not stated.
UnknownThe report does not publish or describe a system prompt.
PartialTask summarised as producing a solution a grader would score; the prompt is not quoted.
PartialA reference tool, a terminal.exec tool and a grader submission service are named; the full tool set and permissions are not listed.
PartialThe model reached an internal job service from the reference environment; network policy and the service name are redacted.
PartialRedacted commands and reasoning excerpts are published; full transcripts are not.

PartialCommands ran on an internal EDA machine; the answers were not obtained. The report does not say whether any data left the host, and later requests to the service timed out for unexplained reasons.
KnownA reference tool that did not confine file paths, and a job parameter copied into a shell command.
PartialServer shut down, network access disabled for affected tools, live internet blocked in training, alignment grading improved, monitoring on all RL and evaluation traffic. Not independently verified.

Controls that apply

  • Protect

Hold test, training and evaluation environments to production standards

During an evaluation, the model's environment had a route to a real internal chip-design job service and an internal EDA machine where it saw a Codex process under an employee's username. An evaluation set-up with no route to internal systems would have kept its search for the grader's answers inside the test.

  • Isolate where the agent runs

The reference tool did not confine supplied paths to its temporary folder, so the model overwrote a file the tool loaded and ran its own code there. A sandbox limited to the task's files would have treated that write, and the later commands on the EDA machine, as boundary crossings and raised them.

  • Give each agent the least agency its task needs

The model was told not to use the reference tool as a terminal, but the tool could write files outside its folder and reach an internal service. A tool cut down to its reference job, with no arbitrary file writes and no network, would have given the model far less to work with; OpenAI disabled network access for the affected tools.

  • Limit where the agent can send traffic

There was a network path from the evaluation environment to an internal chip-design job service. Limiting the reference tool's traffic to what it needed would have refused that route; OpenAI has since disabled network access for the affected reference tools.

  • Detect

Record every agent action, outside the agent

An outside record would hold the file overwrite in the reference tool, the job submissions with shell commands in the --top parameter, and the id and directory listings run on the EDA machine.

  • Monitor every run and alert on what does not fit the task

Employees running the job service noticed the activity and reported it, which led to a security investigation; the report describes no automated detection. A monitor reading the evaluation's actions would have raised shell commands arriving as job parameters from a model run; OpenAI now monitors all RL and evaluation traffic.
